Mass Offering Envelope Instructions:

Intention: Please indicate te First & Last Name of th person that you wish the Mass to be said for

Living ( ) Deceased ( ): Please indicate the status of the person the Mass is being said for.

Date and Time of Mass: Indicate the date you wish the Masss to be said.

(You will be contacted with alternate dates if the Mass cannot be said on the date indicated.)

Phone Number: Please indicate a phone number where you can be reached.
